Printable form to use to notify of intent to homeschool. Individual School Districts may no longer generate their own Intent Forms; they may only offer a standard statewide form made by the Department of Education. The exact wording is as follows: "The Nevada Department of Education shall develop standard Notification of Intent forms, which shall not require any information or assurances other than those required by law." What this means is that the Department of Education will make one form that the districts may hand out to homeschoolers. This does not, however, prevent a homeschooler from using another form of their own choosing.
